Get document change history

/v1/default/banks/:bank_id/documents/:document_id/history

What each change to a document did, newest first: every retain, append and reprocess (facts added and removed), every fact edit, invalidation, restore, delete, retag and per-fact observation clear (observations removed), and every consolidation run (observations created, updated and removed). Observations are rebuilt, not versioned, so this is the only record of the ones a change removed. Ids and counts only, with each id list capped at 100.

Rebuilt from the bank's async operations, which carry these events: it reaches back only as far as operations are kept (HINDSIGHT_API_OPERATION_RETENTION_DAYS, unlimited by default), and a change that ran without an operation is not in it — a synchronous retain (async=false), or a fact edit, delete or retag while auto-consolidation is off. A fact edit, delete or retag is recorded on the consolidation operation it queues. Clearing every observation in the bank is not listed. total, limit and offset count operations, not events.
